What policies or procedures does your institution have in place when deaf students are absent from class or cancel interpreters?

We are working on this as it has become more of an issue over the last year. Traditionally, if a student misses 3 classes in a row without telling us, we let them know that if they miss another consecutive class, we will suspend service until we hear from them. We also ask them if they need additional accommodations or support in getting to class. We are considering the idea of charging the department 50% of the cost for late cancellations as a way to get them to support the student in coming to class.
Another issue has been instructors in some programs changing their meeting days or time without adequate notice. For this we are considering full chargebacks from the department if they make changes that cause a late cancellation.
